Advanced Care Directives

Your Advanced Care Directive is a document which allows you to give directions about your future health care when you are not able to do so yourself.

Your Advanced Care Directive only comes into effect when you are unable to make your wishes known.

Your Advanced Care Directive can easily be changed or revoked as you wish.

It is recommended that you review your Advanced Care Directive at least every two years.

Your Advanced Care Directive can include:

• Medical Treatment you may want to receive,

• Medical Treatment that you don't want to receive,

• Your wishes in relation to Organ Donation,

• Your religious beliefs that may affect your treatment,

• Your Personal Statement
